The single most important factor in staying on schedule is wearing the aligners for the recommended 20 to 22 hours per day. Patients who consistently comply with wear time progress as planned. Those who remove aligners for significantly longer than recommended will extend their treatment timeline, as the planned tooth movements are based on consistent aligner pressure.
For most orthodontic conditions—including mild to moderate crowding, spacing, and common bite discrepancies—Invisalign® achieves results comparable to traditional braces in the hands of an experienced provider. Most patients experience mild pressure or temporary soreness for one to three days when switching to each new aligner. This discomfort is normal and indicates that tooth movement is occurring as planned. It typically resolves within a few days and is managed easily with over-the-counter pain relievers if needed. Most patients find the discomfort much more manageable than they expected.
Invisalign® aligners do not cause the bracket and wire irritation associated with traditional braces. There are no wires to poke the cheeks, no brackets to come loose, and no wire adjustment appointments that can cause temporary soreness. The practical discipline required is to actually remove the aligners before eating and to clean the teeth before replacing them. Patients who eat or drink with aligners in risk staining the trays, trapping food beneath them, and potentially damaging them. The habit of removing, eating, brushing, and replacing becomes second nature quickly for most patients.
Invisalign® progress appointments are typically scheduled every six to ten weeks, though the specific interval depends on the stage of treatment and the complexity of the case. At these appointments, we assess tooth movement, ensure the current aligner is fitting correctly, check for any issues with attachments, and provide the next series of aligners. X-rays are taken periodically to monitor root health and bone response.
Unlike braces, which require monthly wire adjustment appointments, Invisalign®’s less frequent appointment schedule is convenient for patients with busy schedules. Between appointments, patients manage their own progress by switching aligners on schedule and wearing them for the recommended time each day.
After the active aligner series is complete, retainers are required to maintain the achieved results. Teeth have a natural tendency to drift back toward their pre-treatment positions (relapse), and retainers prevent this from happening. Custom clear retainers similar in appearance to Invisalign® aligners are the most common retention option. Fixed (bonded) retainers attached behind the front teeth are another option, particularly for the lower front teeth.
Initially, retainers are worn most of the time and are gradually transitioned to nighttime wear only as stability is confirmed. Long-term nightly retainer wear is the standard recommendation for maintaining orthodontic results indefinitely. Stopping retainer wear allows the teeth to shift, potentially requiring retreatment.
Yes—Invisalign® is commonly used for patients who had orthodontic treatment earlier in life and have experienced relapse (shifting of teeth after braces were removed, usually due to inconsistent retainer wear). These cases often involve more limited movement than original orthodontic treatment and may be suitable for Invisalign® Lite or a shorter comprehensive series depending on the degree of relapse.
Evaluation of the current tooth positions and any prior orthodontic records helps determine the scope of movement needed. Patients with prior orthodontic treatment who have relapsed can often achieve excellent results with a second round of aligner treatment.
